Surgical treatment for lipoedema using liposuction

In recent years, liposuction has established itself as the most effective treatment method for lipoedema. The abnormal fatty tissue is suctioned out using special cannulas. The two most common techniques are tumescent liposuction (TLA) and water-jet assisted liposuction (WAL).

In TLA, a large amount of tumescent solution is first injected into the tissue. This solution contains a local anesthetic for numbing, adrenaline to constrict the blood vessels, and saline solution to cause the tissue to swell. This makes it easier to loosen the fat cells from the connective tissue and suck them out.

WAL, on the other hand, requires smaller amounts of tumescent solution. Instead, a low-pressure water jet is used to gently separate the fat cells from the tissue and suction them out at the same time. Both techniques require an experienced lipedema doctor to avoid injury to the lymph vessels.

In the daily practice of the LipoClinic, water-jet assisted liposuction has proven to be advantageous. New studies and congress reports suggest that WAL has an advantage over other methods.

After surgical lipoedema treatment, it is essential to wear compression garments consistently for at least 6 weeks to reduce swelling and hematomas. Lymphatic drainage and skin care also support the healing process.

What treatment options are there for lipedema?

A basic distinction is made between conservative and surgical treatment. Conservative therapy includes compression, lymphatic drainage, exercise and nutritional advice. Liposuction can provide long-term relief.

This depends on the degree of spread and the area of the body. Often 2 to 3 procedures are carried out in order to treat all affected areas gently. The exact number will be determined in a personal consultation.

You should take it easy for the first few days. Swelling and bruising are normal and usually subside within a few weeks. Sport and strenuous activity should be avoided for approx. 4-6 weeks.

The surgically removed fat cells do not grow back. However, hormonal changes or weight gain can cause abnormal fat to re-accumulate in areas that have not been operated on. Long-term aftercare is therefore advisable.

How can I get rid of my lipedema?

Complex physical decongestive therapy (CDT) is one of the central measures for lipoedema. It consists of manual lymphatic drainage, compression and exercise.

The following measures can help:

  • Low salt diet
  • Sufficient fluids (at least 2 liters a day, ideally water)
  • Herbal teas, e.g. nettle tea, but only in moderation (max. 1-2 cups/day), especially after an operation, to prevent excessive fluid loss.
  • Gentle exercise, e.g. aqua fitness, especially after complete wound healing following an operation, exercise in water is ideal and is well tolerated.

Whether lipoedema patients suffer more from certain vitamin deficiencies has not yet been conclusively clarified scientifically.

A study from May 2024 shows that many patients had low vitamin D levels preoperatively. A further decline was observed after liposuction. Nevertheless, these results do not allow a general statement to be made about all those affected.

Psychological Impacts

Inner heaviness

Proportions don’t match

Lipedema patients often have body proportions that do not match the rest of their body. Typically, they have a slender upper body, while their legs and/or arms are significantly enlarged due to symmetrical fat deposits.

Jumps in caliber

A common feature of advanced lipedema are obvious jumps in size with fat deposits forming around the wrists or ankles. This leads to a noticeable difference in the circumference between the arms/legs and the slimmer hands/feet, which creates what are known as ‘fat cuffs’ or ‘bracelets’.

Bruising

Due to the abnormal fat distribution, patients with lipedema tend to have more permeable capillary walls, which means that tissues can bruise more easily in response to minor injuries or if slight pressure is applied to the affected areas.
